What is Appendicitis?

Appendicitis occurs when the appendix becomes blocked by stool, a foreign body, or infection, leading to swelling and pus formation. This blockage causes severe pain, usually in the lower right side of the abdomen. The only effective treatment for appendicitis is surgical removal of the appendix, known as appendectomy.

There are two main surgical methods — laparoscopic appendectomy (keyhole surgery) and open appendectomy (traditional surgery). Laparoscopic surgery is the preferred method due to its smaller incisions, faster healing, and lower complication rate.

causes

The exact cause of appendicitis is not always clear, but it is usually triggered by:

  • Blockage of the appendix by stool or mucus
  • Bacterial or viral infection in the digestive tract
  • Inflammatory bowel conditions
  • Enlarged lymphoid tissue in the intestine

Risk increases with poor dietary fiber intake, certain infections, or family history of appendicitis.

Common signs and symptoms include:

  • Sudden pain starting around the navel and shifting to the lower right abdomen
  • Loss of appetite
  • Nausea and vomiting
  • Fever with chills
  • Abdominal bloating or swelling
  • Constipation or diarrhea
  • Pain worsening with coughing, walking, or sudden movement

Note: In some cases, especially in children or pregnant women, symptoms may appear atypical. Timely evaluation is crucial to avoid rupture.

Medical diagnosis procedure

Diagnosis of Appendicitis

Diagnosis is based on clinical evaluation and imaging tests, which may include:

  • Physical examination: To identify tenderness in the lower right abdomen
  • Blood tests: To check for elevated white blood cell count (infection)
  • Urine test: To rule out urinary infections or kidney stones
  • Ultrasound or CT scan: To confirm inflammation or rupture of the appendix

Prompt diagnosis is essential as a ruptured appendix can cause life-threatening infection.

Appendicitis Treatment Options

1. Laparoscopic Appendectomy

  • Performed using 2–3 small incisions (0.5–1 cm).
  • A laparoscope (camera) and specialized instruments are inserted to remove the appendix.
  • Offers quicker recovery, minimal scarring, and lower post-surgery pain.
  • Usually recommended for uncomplicated appendicitis.

2. Open Appendectomy

  • Involves a single incision (5–10 cm) in the lower right abdomen.
  • Recommended for ruptured or complicated appendicitis with abscess formation.

3. Conservative (Non-surgical) Management

  • In rare cases of mild infection or when surgery is not immediately possible, antibiotics are used temporarily.
  • Surgery is usually scheduled later to prevent recurrence.
